Das Management der Glykämie bei Typ-II-Diabetes mellitus ist in Abb. 14.5 dargestellt. . Orale synthetische Antidiabetika – Sulfonylharnstoffderivate (PSM) erhöhen den Insulinspiegel im Blut. In diesem Fall nimmt das Körpergewicht zu; Der Gehalt an Laktat und Blutcholesterin ändert sich nicht. PSM ist bei Patienten mit signifikantem oder vollständigem Verlust der Pankreas-β-Zellmasse unwirksam. Nebenwirkungen von PSM: • Hypoglykämie; • allergische Reaktionen (Hautausschlag, Erythem, Juckreiz); • Magen-Darm-Störungen (Anorexie, Übelkeit, Schmerzen); • Verletzung der Blutzusammensetzung (Agranulozytose, Thrombozytopenie); • Hyponatriämie und Flüssigkeitsretention im Körper; • Hepatotoxizität (cholestatischer Ikterus). Glibenclamid (Maninil® 1,75, Maninil® 3,5, Maninil® 5, Glimidstad®, Glibeks, Glidanil®) hat eine ausgeprägte zuckersenkende Wirkung, weshalb seine Ernennung in den frühen Stadien der Krankheit zu hypoglykämischen Zuständen führen kann. Mikronisierte Formen von Glibenclamid (1,75 und 3,5 mg) (Maninil 1,75) weisen eine hohe Bioverfügbarkeit und ein geringes Risiko für die Entwicklung hypoglykämischer Zustände auf. Kombinierte Präparate, die Glibenclamid und Metformin enthalten (Metglib®, Glibomet®, Glucovans®, Gluconorm®, Bagomet Plus®), können aufgrund der Wirkung von Metformip eine Laktatazidose hervorrufen. Glipizid (Movogleken®, Glibenez Retard) hat auch eine ziemlich ausgeprägte zuckersenkende Wirkung. Gleichzeitig stellt dieses Medikament eine minimale Gefahr in Bezug auf hypoglykämische Reaktionen dar. Der angegebene Vorteil von Glipizid beruht auf dem Fehlen einer kumulativen Wirkung, da die während seiner Inaktivierung in der Leber gebildeten Metaboliten keine hypoglykämische Wirkung haben. Derzeit wird eine neue verlängerte GITS-Form von Glipizid verwendet – Glibenesis Retard (Glucotrol XL) (GITS – gastrointestinale therapeutische Form), die mit einer Einzeldosis den optimalen Gehalt des Arzneimittels im Blut sicherstellt. Glycvidonum (Glyurenorm®) ist ein zuckersenkendes Medikament, dessen Ernennung bei Menschen mit Nierenerkrankungen möglich ist. Etwa 95% der erhaltenen Dosis werden über den Magen-Darm-Trakt und nur 5% über die Nieren ausgeschieden. Eine multizentrische Studie über die Wirkung von Glycidon auf die Leberfunktion hat die Möglichkeit seiner sicheren Anwendung bei Menschen mit eingeschränkter Funktion bewiesen. Gliclazid (Gliclada®, Glidiab®, Diabspharm, Diabet® MV, Diabinax, Diabetalong) wirkt sich neben der hypoglykämischen Wirkung positiv auf die Mikrozirkulation, die Blutstillung, einige hämatologische Parameter und die rheologischen Eigenschaften des Blutes aus, was für Patienten mit Typ-II-Diabetes äußerst wichtig ist. Die aufgeführten Wirkungen von Gliclazid beruhen auf seiner Fähigkeit, die Blutplättchenaggregation zu verringern, ihren relativen Disaggregationsindex und ihre Blutviskosität zu erhöhen. Glimepirid (Glemaz®, Glemauno) bindet im Gegensatz zu allen oben genannten Arzneimitteln an einen anderen Rezeptor auf der β-Zellmembran. Die angegebene Qualität des Arzneimittels manifestiert sich in Form seiner Pharmakokinetik und Pharmakodynamik. Bei einmaliger Anwendung von Glimepirid bleibt die konstante Konzentration im Blut erhalten, was erforderlich ist, um eine zuckersenkende Wirkung für 24 Stunden sicherzustellen. Merkmale der Assoziation von Glimepirid mit dem Rezeptor tragen zum raschen Einsetzen der zuckersenkenden Wirkung bei, und die Dissoziation mit demselben Rezeptor eliminiert praktisch das Risiko hypoglykämischer Zustände. Das kombinierte Präparat, das Glimepirid und Metformin (Amaril® M) enthält, kann nur bei Patienten mit normaler Nierenfunktion angewendet werden, da das Risiko einer schweren Schleimhautobstruktion bei Metformin bei Patienten mit eingeschränkter Nierenfunktion höher ist. Orale synthetische Antidiabetika – Biguanide den Insulinspiegel im Blut normalisieren oder sogar senken. Im Gegensatz zu PSM tragen Biguanide zu einer Verringerung des Körpergewichts und des Cholesterins im Blut bei. Gleichzeitig steigt der Laktatgehalt im Blut, es kommt zu einer Azidose. Metformin (Glyformin Prolong®, Glucofage® Long) wird verwendet in folgenden Fällen: • Fettleibigkeit bei Patienten mit NIDDM; • Diabetes mellitus bei Menschen über 40 Jahren mit einer Krankheitsdauer von nicht mehr als 5 Jahren und einem Insulinbedarf von 30 bis 40 Einheiten pro Tag; • mangelnde Wirkung der Monotherapie von Sulfonylharnstoffderivaten; • Perspektiven für eine kombinierte Behandlung mit PSM. Nebenwirkungen oraler (synthetischer) Antidiabetika – Biguanide: • Magen-Darm-Störungen (Anorexie, Übelkeit, Erbrechen, Durchfall, metallischer oder bitterer Nachgeschmack); • Gewichtsverlust; • Laktatazidose. Gegenanzeigen zur Verwendung von Biguaniden: • insulinabhängiger Diabetes mellitus (IDDM); • diabetische Ketoazidose; • Erkrankungen der Leber und Nieren; • Lungenherzinsuffizienz; • Schwangerschaft, Stillzeit; • Hyperlaktatämie; • akute Infektionskrankheiten; • Alkoholismus. Die kombinierten Medikamente Metformin + Saxagliptin (Combogliz Prolong®) und Metformin + Vildagliptin + (Galvus Met) kombinieren zwei hypoglykämische Medikamente mit einem zusätzlichen Wirkmechanismus zur Verbesserung der Blutzuckerkontrolle bei Patienten mit Typ-II-Diabetes mellitus. Gliptine (Saxagliptin und Vildagliptin) sind Inhibitoren der Deeptidase-4 (DPP-4). In Reaktion auf die Nahrungsaufnahme aus dem Dünndarm werden Inkretinhormone in den Blutkreislauf freigesetzt, wie z. B. das Glucagon-ähnliche Peptid-1 (GLP-1) und das Glucose-abhängige insulinotrope Peptid (HIP). Diese Homons tragen zur Freisetzung von Insulin aus den Pankreas-β-Zellen bei, werden jedoch innerhalb weniger Minuten durch das DPP-4-Enzym inaktiviert. Gliptine, die DPP-4 kompetitiv hemmen, erhöhen die Konzentration von Inkretinhormonen, fördern die Freisetzung von Insulin und verringern die Konzentration von Glukose im Blut. Metformin ist ein Vertreter der Biguanidklasse. Combogliz Prolong und Galvus Met werden bei Typ-II-Diabetes mellitus in Kombination mit Diät und Bewegung angewendet, um die Blutzuckerkontrolle zu verbessern. Das kombinierte Medikament Metformin + Rosiglitazon (Avandamet) enthält zwei hypoglykämische Inhaltsstoffe mit komplementären Wirkmechanismen. Rosiglitazon, eine Thiazolidindion-Klasse, erhöht die Empfindlichkeit des Zielgewebes gegenüber Insulin. Metformin wirkt hauptsächlich durch die Verringerung der Produktion von endogener Glukose in der Leber und wird bei Typ-II-Diabetes mellitus angewendet. Im Gegensatz zu Metformin verursachen Thiazolidindione eine Zunahme des Körpergewichts. Dieses Phänomen ist dosis- und zeitabhängig. Flüssigkeitsretention ist eine häufige Nebenwirkung von Thiazolidindionen. Dies trägt zur Gewichtszunahme, zum Auftreten von peripheren Ödemen, Herzinsuffizienz sowie zu Anämie aufgrund von Hämodilution bei. Gegenanzeigen für die Verwendung aller Medikamente, die die Empfindlichkeit peripherer Gewebe gegenüber Insulin erhöhen, sind Typ-I-Diabetes, Schwangerschaft und Stillzeit. Darüber hinaus sind Kontraindikationen für die Ernennung von Metformin alle Faktoren, die für die Entwicklung einer Laktatazidose prädisponieren: chronisches Nierenversagen, Herzinsuffizienz, Lebererkrankung, Alkoholmissbrauch, schwere Körpererkrankungen, begleitet von Hypoxie und (oder) Azidose, Operation, Alter über 80 Jahre. Thiazolidindione sind in den NYHA-Klassen III und IV sowie bei einem 2,5-mal höheren ALT-Wert als der Obergrenze des Normalwerts kontraindiziert. Acarbose (Glucobay®) hemmt das Enzym α-Glucosidase, das von Bürstensaumzellen im proximalen Teil des Dünndarms ausgeschieden wird. Acarbose verbindet sich kompetitiv, reversibel und dosisabhängig mit α-Glucosidase und verhindert den Abbau komplexer diätetischer Kohlenhydrate und Disaccharide wie Stärke, Dextrin und Saccharose. Dieser Effekt beginnt in den ersten 10 bis 15 Minuten nach der Einnahme und dauert 4 bis 6 Stunden. Wenn Produkte, die Saccharide enthalten, in den Verdauungstrakt gelangen, wird normalerweise die Hälfte der Glukose in der ersten Stunde und der Rest in der zweiten Stunde absorbiert. Bis zu 80% der postprandialen Insulinsekretion sind auf die Wirkung von Inkretinen, einschließlich GLP-1, zurückzuführen. Bei gesunden Menschen ist GLP-1 eines der stärksten Stimulanzien für die Insulinsekretion. Exonagider Glucagon-ähnlicher Peptidrezeptoragonist wird zur Behandlung von Typ-II-Diabetes in Kombination mit Metformin verwendet. Exenatide (Lösung zur subkutanen Verabreichung) wird bei Patienten mit schwerem Nierenschaden und Nierenversagen nicht empfohlen. Die primäre Resistenz gegen orale Hypoglykämika (PSP) ist eine Erkrankung, bei der im ersten Monat nach Beginn der Einnahme von Hypoglykämika trotz maximaler Dosis keine Wirkung der Behandlung auftritt. In diesem Fall ist die Verwendung von Insulinpräparaten erforderlich, da die verbleibenden β-Zellen der Bauchspeicheldrüse des Patienten nicht genügend Insulin produzieren, wenn sie durch orale LP stimuliert werden. Eine sekundäre Resistenz gegen PSP tritt bei einer bestimmten Anzahl von Patienten mit NIDDM mehrere Jahre nach Beginn der Behandlung auf. Dies tritt als Folge einer fortgesetzten Dysfunktion der Pankreas-β-Zellen auf. In diesem Fall ist eine sofortige Verabreichung von Insulin an den Patienten erforderlich. Moderne Insuline und Analoga, die zur Anwendung bei Patienten mit Typ-II-Diabetes empfohlen werden, sind in der Tabelle aufgeführt. 14.4. Eine Langzeit-Insulintherapie für NIDDM ist angezeigt für: schwere Spätkomplikationen bei Diabetes (hohes Maß an Retinopathie, schwere periphere Neuropathie, insbesondere schmerzhafte Form, progressive Nephropathie). Tabelle 14.4. Moderne Insuline und Analoga werden zur Anwendung bei Patienten mit Typ-II-Diabetes empfohlen.
